Authorization for JP Auto Collision Repairs: This contract represents the entire legal agreement between JP Auto Collision and our customers. No other promises or agreements outside of this contract and subsequent repair authorizations shall be considered valid. No services outside of those described in this auto repair contract will be performed by the company without prior written, phone call, or texting approval from the customer. This contract is binding upon both parties and their assigns or successors, including subsequent title holders for the vehicle being repaired. I/we hereby appoint JP Auto as my/our agent and attorney-in-fact to endorse any check or draft issued by the insurance carrier mentioned above for the purpose of covering all or part of the repairs to my/our automobile. I/we acknowledge and confirm any action taken by said agent and attorney-in-fact in this regard. JP Auto can only provide an estimated completion date for the repairs on my vehicle. Delays due to parts availability, insurance re-inspections, supplemental or hidden damages may occur and can affect the completion time. JP Auto Collision is not responsible for providing a complementary loan vehicle while my vehicle is in the shop for repair and service work. It is my sole responsibility to arrange alternate transportation. I have removed all valuables from the vehicle. I understand that I am responsible for any deductible adjustment, depreciation, or betterment amounts if my insurance company fails to pay for other labor, parts, or material costs necessary to restore my vehicle to its pre-accident condition as required by state law. I authorize the repair work to be done, along with the necessary materials, and grant JP Auto Collision and its employees permission to operate the described vehicle for testing, inspection, and transport. I acknowledge the mechanic's lien on the vehicle to secure the amount of repair. I understand that the vehicle is subject to repossession if payment for the repair is stopped, dishonored, or the account is closed. I certify that I have the legal right to authorize repairs for the listed vehicle. Full payment must be made before the car is returned. JP Auto Collision progresses through three stages: Vehicle Analysis, Additional Damage, and Insurer Estimate. Any repairs outside of this contract will require written authorization. JP Auto has no formal relationship or obligation to any insurer. Their sole responsibility is to me, and they are not obligated to communicate with or assist insurers or their agents. I acknowledge the risks associated with using aftermarket parts, including lease or finance violations, increased depreciation, lost vehicle value, and potential voiding of manufacturer warranties. JP Auto Collision will not use any parts for repairs unless approved by customers. If a vehicle comes from a different shop for service or diagnosis, JP Auto Collision is not liable for pre-existing conditions or any new problems arising from not performing recommended repairs. By signing below, I acknowledge that I have read and understood the information above, and I authorize JP Auto Collision to perform the necessary repairs on my vehicle.
